Registering with Us...

To register with the surgery please come in person between 10am and 12.30pm if possible when our Admin Assistant will be happy to help you. However if these times are inconvenient our Receptionists will be able to help you register at other times.  Please bring your medical card, if you have one. If you do not have one you will be given a registration form to complete. We also require two forms of identfication, one photo identification for example passport or photo driving licence and proof of address for example a utility bill or bank statement dated within the last three months.

Our reception staff will be pleased to book you a New Patient Check and answer any queries you may have.
